DATA SUBJECT APPLICATION FORM FOR APPLICATIONS TO BE MADE BY THE PERSONAL DATA OWNER TO THE DATA CONTROLLER

GENERAL EXPLANATIONS:

Law No. 6698 on the Protection of Personal Data (“Law”) stipulates that personal data owners (“Data Owner”), who are defined as the relevant person in the Law No. 6698 on the Protection of Personal Data (“Law”), can exercise the rights granted in Article 11 of the Law regarding their personal data and must apply to the Data Controller to exercise these rights.

Pursuant to paragraph 1 of Article 13 of the Law; Applications to be made to the data controller Analiz Kariyer Danışmanlık Eğitim Özel İstihdam Ve İnsan Kaynakları Turizm Bilişim Yazılım Ticaret Limited Şirketi (“Analiz Kariyer”) regarding these rights must be submitted by the following methods.

APPLICATION ROUTE:

In accordance with paragraph 1 of Article 13 of the Law No. 6698 and the Communiqué on the Procedures and Principles of Application to the Data Controller No. 30356 and dated 10.03.2018, you can submit your request to exercise your rights mentioned above in Turkish and in writing or by using your registered electronic mail (KEP) address, secure electronic signature, mobile signature or your e-mail address available in Analiz Kariyer records. Analiz Kariyer reserves the right to verify your identity before responding.

In your application;

• Your name, surname and signature if the application is in writing,
• For citizens of the Republic of Turkey, your Turkish ID number; if you are a foreigner, your nationality, passport number or ID number, if any,
• Your residential or workplace address for notification,
• Your e-mail address, telephone and fax number, if any,
• Your request subject,

and the relevant information and documents, if any, must also be attached to the application.

You can submit your applications that you want to make in writing by filling out this form and attaching the necessary documents to its annex to the authorized personnel at the general directorate of Analiz Kariyer as the data controller. (Address: Ümit Mah. 2545 Sk. No: 11 Çankaya, Ankara)

You can send your applications via e-mail to info@analizkariyer.com e-mail address.

You can make your applications via KEP to our ……………. KEP address.

According to the nature of this form and your request, information and documents must be provided to us completely and accurately. If the requested information and documents are not provided as required, there may be problems in the full and qualified execution of the researches to be carried out by Analiz Kariyer based on your request. In this case, Analiz Kariyer declares that it reserves its legal rights. For this reason, the relevant form must be sent completely and in a way to include the requested information and documents according to the nature of your request.

In order to ensure the security of your personal data, within seven (7) days from the date your application for information is received by Analiz Kariyer, Analiz Kariyer may contact you to confirm that you are the data owner and may request some information and documents from you in this regard.

In case the requested information and documents are incomplete, the information and documents will have to be completed and submitted to us upon our request. The thirty (30) day period specified in Article 13/2 of the Law regarding the finalization of the request will be suspended until the information and documents are fully submitted to us.
